Ankle Fractures Flashcards
What is an Ankle #? (2 things)
- # of any malleolus (lat / med / post)
- +/- disruption of syndesmosis
What is the Syndesmosis?
Strong fibrous structure where Tibia + Fibula join
What is the syndesmosis made up of? (3 things)
- Ant. Inf. Tibiofibular Lig (AITFL)
- Post Inf. Tibiofibular Lig (PITFL)
- Intra-osseous memb.
How are Ankle # anatomically described? (4 things)
- Isolated lat malleolar #
- Isolated med malleolar #
- Bimalleolar # (lat + med)
- Trimalleolar # (lat + med + post)
What is the classification system of LATERAL malleolar Ankle #s?
Weber classification
What are the groups in Weber classification of lat malleolar #s? (3 things)
- Weber A = # below syndesmosis
- Weber B = # @ level of syndesmosis
- Weber C = # above syndesmosis
Which Weber classification # needs surgical fixation?
Weber C bc more likely to have syndesmosis ligament disruption –> more ankle instability
What are the clinical features of Ankle #s? (3 things)
- Pain
- Gross swelling (can delay surgery)
- Can be very deformed open # w. neurovasc compromise
What should you LOOK for in examination of an Ankle #? (4 things)
- Swellling (1+ malleolus)
- Tenseness
- Tissue compromise
- Deformities (dislocation of tibiotalar joint / gross displacement)
What should you FEEL for in examination of an Ankle #? (2 things)
- Bony tenderness (Ottowa Rules)
- Pulses + sensation
What MOVEMENTS should you assess in examination of an Ankle #? (4 things)
- Inversion
- Eversion
- Plantar flexion
- Dorsiflexion
What rules indicate the need for ankle XR?
Ottowa Rules
What are the Ottowa Rules? (3 things)
Pt needs ankle XR if has pain + one of:
- Bony tenderness @ lat. malleolar tip to post. border of fibula
- Bony tenderness @ med. malleolar tip to post. border of tibia
- Inability to walk 4 weight bearing steps
What imaging should be done for Ankle #s?
- Plain XR (AP + lat)
- CT for complex #
What should you check for in XR of Ankle #s? (2 things)
- Check joint space for uniformity
- Ensure no evidence of talar shift
