Define shock
a state of insufficient blood supply to the tissues of the body as a result of problems to the circulatory system.

life-threatning dysfuntion caused by disregulted response to infection -overstimulation of innate immune response
Define septic shock
shock (inadequate blood supply to tissues) as a result of overreaction from innate imuune system
what medication is given to a patient with sepsis to maintain blood pressure
noradrenaline and/or dopamine
What are the clinical manifestations of sepsis?
Hypotension
fever
High RR
oedema?
Why do patients who develop sepsis have hypotension?
increased capillary permiability
results in fluid shift out of vessels, thus causes intravascular volume depletion
What does SOFA stand for?
sequential organ failure assessment score
What are the three elements which make up a qSOFA?
Alter mental state
Respiratory rate
Hypotension
At what news score should a patient who has the clinical manifstations of sepsis should be querid for?
greater or eual to 5
What is the map required for sufficient organ perfusion?
great or equal to 65
How is map calculated?
((diastolic x2 ) + systolic ) / 3
When a patient is ?sepsis, what are the steps that should be taken within the “golden hour”?
- adminsiter high flox oxygen if sats are low. use reservoir mask
- Take and send blood cultures
- Administer broad spectrum IV antibiotics
- If patient is hypotensive, administer IV crystalloid fluids.
- Check lactate levels
- If patient remains hyp, administer vassopressers
- Monitor urine output hourly
what lactate levels suggest a sign of anarobic metabolism?
greater than 4
At what Hb level would one consider administering a blood transfusion?
lower than 70 Hb
What is considered “normal” urine output?
0.5-1ml/kg/hr
