What is sepsis?
Sepsis is an infection with a systemic response
What are the criteria for sepsis
- Must have a suspected infection
- Must have one or more red criteria
- Two or more yellow criteria
What are the red criteria!
- Objective altered mental state
- Resp rate >25 (or oxygen required)
- Heart rate >130
- Systolic BP<90 (or 40 below normal)
- Urine output <0.5ml/kg/hr or not passed any in 18hrs
What are the orange criteria?
- History of altered mental state
- RR >20
- Heart rate >20
- Systolic >100
- Urine output <1ml/kg/h
What are the history parameters for sepsis?
- Deterioration in functional ability
- Rigors
- Impaired immunity/immunosuppression
- Recent surgery/ trauma
What are the complications of Sepsis?
Look for end organ dysfunction
How does it effect the circulatory system?
- Hypovolaemic/ shock: SBP<90
His does it affect the lungs?
- AARDS
2. inc oxygen requirements to maintain SATs >90%
How does sepsis affect the kidneys?
- AKI
2. Urine Output <0.5/kg/h or creatinine>175micro moles/L
How dies sepsis affect the liver?
- Hyperbilrubinaemia
2. Bilirubin >35 mmol/l
How does sepsis affect coagulation?
- Disseminates intravascular coagulation: widespread activation of clotting cascade in small blood vessel.
- Leads to multiple organ failure
- Platelets <100×10/9/L or INR >1.5
How does sepsis affect the brain?
- Encephalopathy

What is septic shock?
Sepsis with refractive hypo tension despite adequate fluid resuscitation
How is sepsis treated?
Sepsis 6 within 1 hour!

What are the 3 things ‘in’ whilst treating sepsis?
- Oxygen
- Fluids
- Antibiotics
