Ischemia at rest
changes in T wave or ST segment that are out of place; T wave usually has same polarity as QRS
Ischemia during exercise
ST segment depression;
up-sloping: cause of many false positives
Down-sloping: almost certainly ischemia
Horizontal: likely associated with ischemia
Acute STEMI infarction
elevated ST segment; may have t wave inversion; greater ST displacement = greater ischemic area;
Indication of old infarction
nonsignificant Q’s may be found in VI, III, AVR alone; at least 1/3 the height of the R wave; must be new; usually in more than one lead
Old Inferior infarction
Significant Q’s in II, III, AVF
Old anterior infarction
Significant Q’s in 1, V1, V2, V3
Old lateral infarction
Significant Q’s in V4, V5, V6
BBB on old infarction
all results would be invalid
