secondary/tertiary hyperparathyroidism Flashcards
1
Q
what is secondary hyperparathyroidism
A
- low calcoum or high phosphate
- reactive hyperplasia of parathyroid glads
- PTH secretion is high
2
Q
Name 3 causes of secondary hyperparathyroidism?
A
- chronic kidney disease
- impaired phosphate secretion
- high phosphate –> high PTH
- reduced vit D -> reduced calcium reabsorbtion –> High PTH
2. Vitamin D deficiency
3. Malnutrition
3
Q
2nd HPT - lab results for
- calcium
- phosphorous
- ALP
- PTH
A
- Calcium = normal/low
- Phosphate = normal/high
- ALP = high
- PTH = high
4
Q
Mx of 2nd HPT
A
- treat underlying cause e.g. CKD
- tx hyperphosphatemia
- restrict phosphate in diet - Vitamin D supplement - ergocalciferol
5
Q
What is the definition of ‘tertiary hyperparathyroidism’
A
- hypercalcaemia from untreated sHPT
- refractory/autonomous PTH secretion
- hypercalcaemia
