what is hyperparathyroidism?
A
overproduction of PTH by parathyroid chief cells

how do most patients present?
A
majority of patients with primary hyperparathyroidism are assymptomatic

clinical features : kidneys
A
- nephrocalcinosis
2. polyuria and polydipsia

A
- bone pain
- peudogout
- skull - decalcification (salt and pepper skull)

- lack of appetite
- nausea and constipation
- gastric / duodenal ulcers

- anxiety

What are the lab results for each of the following;
- calcium
- phosphate
- alkaline phosphotase
- PTH

- calcium is high
- phophate is low
- ALP is high
- PTH is high

Max - First line
A
- surgical
para/total parathyroidectomy

When is surgical management indicated?
A
- symptomatic patient
2. assymptomatic but high calcium/nephtolithiasis etc

Mx - non surgical treatment
A
- calcimimetics
2. bisphosphonates - slows down degretation of bone
