Signs and symptoms of CV or pulmonary disease
- pain/discomfort in chest, neck, jaw, arms
- shortness of breath at rest or with mild ex
- dizziness
- ankle edema
- Orthopnea
- Claudication
- Heart murmur
New guideline aims
Aim to identify individuals:
1) who should receive medical clearance before exercising
2) with clinically sig diseases who may benefit from supervised ex
3) with medical conditions that may require exclusion from ex programs
What is a key change with the new guidelines?
No clearance needed for moderate intense ex of not diagnosed with disease
No clearance needed for moderate intense ex of diagnosed indiv. who regularly exercises
Dose response and exercise
ABC baseline activity status
- Biggest improvement seen in the least fit groups
- If improve exercise capacity by 1.5METS then mortality/morbidity incidence from CVD is halved
Guidelines for exercise?
Children - 5x60 min
Young adults - 30min most days
Older adults - 30min most days + strength and balance 3x/wk
ACSM’s old participation health screening recommendations
1) emphasis on he need for medical evaluation
2) use the term risk classification to group as low, mod, high based on CVD risk factors etc
3) emphasise identifying those known with disease
4) adopt the AACPR risk stratification scheme
5) support the public health message
Difference from old guidelines
- continue to encourage CVD risk factor assessment
- simply the pre-participation health screening process in order to remove unnecessary and unproven barriers to adopting a physically active lifestyle
What is a MET?
Metabolic Equivalent of Task
Defined as the ratio of metabolic rate during a specific physical activity to a reference metabolic rate
= to 3.5ml VO2•kg-1•min-1
How can intensity be measured?
- speed of exercise
- light/mod/vig
- in relation to capacity
- HR
- amount of kcal/min
- VO2 used or METS
- RPE
How do we quantify PA?
IPAQ short form
- Walking
- Moderate-intensity
- Vigorous-intensity activities
Provides separate scores for each activity
