Red flags for lower back pain
- age < 20 years or > 50 years
- history of previous malignancy
- night pain, history of trauma
- systemically unwell e.g. weight loss, fever
What are the features of facet joint pain?
Pain worse in the morning and on standing
On examination there may be pain over the facets. The pain is typically worse on extension of the back
Spinal stenosis
Narrowing of the spaces within your spine, which can put pressure on the nerves that travel through the spine.
Features of spinal stenosis
Usually gradual onset
Relieved by sitting down, leaning forwards, and crouching down
Clinical examination is often normal
What is Ankylosing spondylitis?
Inflammatory arthritis that, over time, can cause some of the bones in the spine (vertebrae) to fuse. This fusing makes the spine less flexible and can result in a hunched posture. If ribs are affected, it can be difficult to breathe deeply.
Features of Ankylosing spondylitis
Typically a young man who presents with lower back pain and stiffness
Stiffness is usually worse in morning and improves with activity
Peripheral arthritis (25%, more common if female)
Features of peripheral arterial disease
Pain on walking, relieved by rest
Absent or weak foot pulses and other signs of limb ischaemia
Sciatica
pain felt along the sciatic nerve, which runs from your lower back, down through the buttock, hamstrings and into the lower leg.
Causes of sciatica
Herniated or slipped disk Degenerative disk disease Spinal stenosis Spondylolisthesis Osteoarthritis Tumours, trauma
