Diagnostic criteria for SSOPD
Abnormalities in 1+ of 5 domains:
- Delusions
- Hallucinations
- Disorganized thinking (speech)
- Grossly disorganized or abnormal motor behavior (including catatonia)
- Negative symptoms
Define delusions
“Fixed beliefs that are not amenable to change in light of conflicting evidence”
Persecutory delusion
I’m going to be harmed, harassed, etc. by a person, organization, or other group
Referential delusion
certain gestures, comments, environmental cues, etc. are directed at me
Grandiose delusion
I have exceptional abilities, wealth, or fame
Nihilistic delusion
a major catastrophe is going to occur
Somatic delusion
preoccupied with health or organ functions
Erotomanic delusion
false belief someone is in love with me
What are “bizarre” delusions? Thought withdrawal delusion
Thought withdrawal delusion: government stole my thoughts
Thought insertion: alien thoughts were put in my mind
Delusions of control: my body or actions are being determined by a force outside of me
Non-bizarre vs bizarre delusions
non-bizarre may actually happen in real life
bizarre are clearly impossible, not derived from ordinary life experiences
Define hallucinations
Sensory perception without an external stimulus and not under voluntary control
What is catatonic behavior?
marked decrease in reactivity to the environment
may include bizarre postures, negativism (resist instructions), mutism, stupor, repeated stereotyped movements
What are negative symptoms?
= deficits of normal emotional response or thought process; not responsive to meds

Delusional disorder - mixed type
no 1 delusional theme predominates

Brief psychotic disorder criteria
presence of 1+ (1 must be 1, 2, or 3)

Duration: less than 1 month
Schizophreniform disorder diagnostic criteria
2+ present (1 must be 1, 2, or 3)
- Delusions
- Hallucinations
- Disorganized speech
- Grossly disorganized or catatonic behavior
- Negative symptoms
Duration: 1-6 months
2/3 of Schizophreniform disorders will progress to ________.

2+ present for significant portion of 1-month period (1 must be 1, 2, or 3)

Duration: at least 6 months

Diagnostic criteria of Schizoaffective Disorder
Uninterrupted period of major mood episode (MDD or manic) concurrent with criteria of Schizophrenia
Delusions or hallucinations for 2+ wks in absence of mood d/o
2 types of Schizoaffective disorder
Bipolar type
Depressive type
Define Substance/Med-induced psychotic disorder
delusions and/or hallucinations in response to substances or meds
Most common causes of psychotic disorder due to another medical condition
untreated endocrine, metabolic disorders, autoimmune disorders, temporal lobe epilepsy
Catatonic associated with another mental disorder
3+: stupor, catalepsy, waxy flexibility, mutism, negativism, posturing, mannerism, stereotypy, agitation, grimacing, echolalia, echopraxia
