Approach to Hypoxia
Pt not breathing or airway obstruction (obstruction, opioids, decreased GCS)
A = look for obstruction. Airway manouvres and adjuncts.
B = look, listen and feel
C = IV lines, BP, pulse. Resucitate with fluids.
D= AVPU
E= Equipment for transfer
F= Notify family
G= glucose
H= hypothermia, cool to 32-34 degrees post cardiac arrest
I = ECG, CXR, Bloods
Life threatening tachyarryhthmia
Wide complex e.g. VT, torsades
If unstable- shock and give amiodarone 300 mg IV.
If stable then:
- give Amiodarone 300 mg IV over 20 minutes
- if torsades then give 2 g og Mg over 10minutes
Non life threatning tachyarryhtmia Mx
Narrow complex e.g. SVT, AF. If rate 150 than flutter.
- vagal manouvres
- adenosine 6 mg IV
Life threatening bradyarrhythmia
- Give atropine then adrenaline
- Unstable, TCP
- If BB/CCB overdose, give glucagon
Aystole/PEA
CPR 30:2 for 2 minutes IV adrenaline ASAP Assess rhythm Non shockable - recommence CRP and give adrenaline in second round Give amiodarone on 3rd shock
VF/pulseless VT
Start CPR 30:2 for 2 minutes Attach defibs Assess rhythm Shock if shockable Continue CPR
