Scenario
Delay when overhead destination
A
- INFO - ask reason for delay and expected approach time
- INFO - weather conditions at destination and alternate
- COMMIT - probably to destination, AWARE that a later diversion will lead to a landing below FRF

Lose radio contact
A
- Try alternative/previous frequency and 121.5
- If no success, squawk 7600, follow local radio failure procedures
- Most likely continue the approach, transmitting blind on the tower frequency

Engine fire on ILS approach, RVR deteriorating
A
- First priority is flying the airplane, continue the approach as stable as possible (aviate, navigate)
- Carry out memory items
- Make mayday call
- Continue approach below minima as fire risk is greater than risk of breaking legal minima

Captain incapacitated, RVR 550m, BKN010
A
Continue approach to CAT I minima, aware of possibility of go around if BKN clouds are around at DA.

On short final ILS get told to go around by ATC, but there is TS in G/A path
A
- Reply UNABLE go around path due thunderstorm, request alternate vectors
- If no response, make own go around route, considering MSA and local knowledge, avoiding TS, transmitting actions

Captain on approach with XWC over limits
A
- Point out to the captain “crosswind above limits”
- If he says continue, reply “SOP is go around”
- Continue to take care of responsibilities, focus on flying the aircraft
- Repeat “crosswind over limits, we should go around”
- Report the situation later

Overhead Germany on way to London, fuel insufficient to reach alternate, continue?
A
- INFO: Flight time remaining? Weather at destination? # runways?
- If flight time > 4 hours, at least 2 runways and good weather (2000ft, 5km) can continue

Cabin crew in conversation with passenger, getting upset
A
- If no checklists to do, consider intervening to assist
- If checks to do, try to draw the attention of senior cabin crew or captain
- When time enables consider speaking to the crew and/or passenger to attempt to resolve the situation

Captain smoking during turnaround
A
- Remind him of the rules and safety concerns.
- If he continues, say “I’ve got no choice but to report this, could you please just put the cigarette out and we’ll carry on?”
