Scenario Evaluations Flashcards
Assess your partner’s hands (not nails) for colour (2 findings), temperature, and fingernails for capillary refill.
Provide findings.
- capillary refill is normal, returns less than 3 secs
- no discolouration, colour is continuous with rest of the arms
- No massess, lumps, lesions
- skin is warm to touch
Assess your partner’s skin turgor and assess moisture, texture, and thickness on arms. Report 1 finding for each.
- skin returns to original position, no skin tenting,
- skin is dry
- skin is soft and smooth
- normal thickness throughout
Assess your partner’s fingernails for clubbing and nail condition, colour, texture, and consistency. Provide
Findings.
- no clubbing on fingernails, normal nail angle
- no lesion/ masess or swelling on nail bed
- nail bed is pink in colour
- nails are intact and smooth with no cracks or ridges
- nails are consistent bilaterally
Inspect and palpate elbows. Provide 2 findings for inspection and 2 findings for palpation.
- symmetrical with no deformities,
- colour is a slightly darker than the rest of the arm
- no masses or swelling
- elbows are warm to touch bilaterally
Assess for ROM of shoulders and say the name of the movements out loud. Provide 3 findings.
- Smooth movements,
- full Active ROM bilaterally.
- No obvious pain, tenderness, restrictions, crepitation bilaterally.
Conduct manual muscle testing of shoulders and say names of movements out loud. Provide findings.
- No pain or tenderness reported
- Grade of 5 which is active movement, with full resistance equal bilaterally
Inspect the posterior thorax. Provide 6 findings.
- Thoracic cage is symmetrical with no signs of rib deviation, - no signs of discoloration,
- no masses, lesions or deformities.
- Straight spinous process appears straight,
-1:2 anteroposterior to transverse diameter, - shoulder and scapula symmetrical
Palpate the posterior chest wall (provide 3 findings) and test for symmetrical expansion (provide 1 finding).
- Posterior chest wall has no deformities,
- no lesions or masses or swelling,
- skin is warm to touch,
- no pain and no crepitus.
- Chest expansion is symmetrical
Inspect the external nose (provide 4 findings) and internal nose (provide 4 findings).
-External nose is symmetrical,
-midline on the face,
-no obvious deformities/ masses/ or swelling,
-colour is consistent with the rest of the face.
-Internal nose has no bleeding, -no masses/ deformities/ swelling,
-septum is midline with no deviation,
-internal mucosa of the nose is pink in colour,
-no foreign bodies
Auscultate the carotid arteries (provide findings) and palpate the carotid arteries (provide 2 findings).
-Upon auscultation - no bruit or adventitious sounds present bilaterally,
- Upon palpation - carotid artery has normal rhythm,
- 2+ force which is normal bilaterally.
- With a quick upstroke and gradual downstroke
Inspect the jugular pulsation, note location, and provide 4 findings.
- While inspecting, right external jugular vein is superficial to the sternocleidomastoid muscle
External Jugular vein is flat with no distention (bulging). - Internal jugular vein is also flat with no bulging.
- Jugular pulsation is not visible meaning central venous pressure is not elevated. Or double diffuse pulsation of right internal jugular vein at sternal notch
Auscultate and name all four cardiac valves with the bell or the diaphragm and provide 2 findings.
- First auscultate using the bell, → Aortic valve, Pulmonic valve, tricuspid valve, mitral valve
- No murmurs or adventitious sounds present,
- S1 and S2 sounds are clear and distinct with no split
Auscultate the abdomen for vascular sounds (name locations) and provide findings.
- No vascular sounds meaning unobstructed blood flow,
- no bruit heard over aortic, renal, iliac and femoral arteries,
Inspect legs (provide 4 findings) and palpate legs for temperature (provide findings).
- Legs are symmetrical,
- hair is evenly distributed bilaterally,
- colour is continuous on the rest of the legs - no signs of discoloration,
- no masses, swelling observed. - Legs are warm to touch bilaterally.
- Spider veins possible or absent
Palpate dorsalis pedis pulses and posterior tibial pulses and provide 2 findings.
- Pulse are present
- 2+ force equal bilaterally
Inspect the abdomen and provide 5 findings.
- Abdomen is symmetrical,
- the umbilicus midline, and
- the abdomen is rounded with no distention.
- No masses or swelling.
- Abdomen colour is continuous with the rest of the body
- shows no sign of discolouration.
Auscultate the abdomen for bowel sounds and provide 3 findings.
- Normoactive bowel sounds are heard in all four quadrants. - High pitched
- gurgling sounds
Perform light palpation of the abdomen and provide 3 findings.
- Abdomen is soft to touch,
- no masses swelling or rigidity. - No pain reported.
Test the sensory function of cranial nerve V (trigeminal) and name the areas. Report findings
- Sensations are intact on ophthalmic, maxillary and mandibular areas bilaterally.
Test extraocular eye movement. Report findings.
- Smooth and coordinated movement of eyeballs. No double vision and client’s gaze is midline
Conduct finger-to-nose test and finger-to-finger test. Report findings.
- Movement is accurate and steady in motion
