A multiple member injury report must be forwarded to
- Chief of Operations
- 3 or more members suffer injuries at the same incident resulting in medical leave
- OOD completes report
- If officer on leave more than 30 days than a REGULARLY assigned officer shall complete report
- Officer NOT responsible to diagnose the injury
- Once report completed company commander reviews & forwards
