a multi-system granulomatous disorder
Acute sarcoid presentation
erythema nodosum
+/- polyarthalgia
(usually resolves spontaneously)
Sarcoid pulmonary sx
Dry cough,
progressive SOB
chest pain
progressive deterioration in lung function (10-20%)
CXR findings of sarcoid
bilateral hilar lymphadenopathy (BHL) ± pulmonary infiltrates or fibrosis
Staging of sarcoid
Stage 0: normal
Stage 1: BHL
Stage 2: BHL + peripheral pulmonary infiltrates
Stage 3: peripheral pulmonary infiltrates alone
Stage 4: progressive pulmonary fibrosis; bulla formation (honeycombing); pleural involvement.

conjunctivitis
keratoconjunctivitis sicca
glaucoma
Neurological sx of sarcoid
Bell’s palsy
Neuropathy
Meningitis
Space-occupying lesion
Dermatological sx of sarcoid
lupus pernio

arrythmias
Renal sx of sarcoid
renal stones (hypercalciuria)
Diagnostic test for sarcoid
non-caseating granulomata on tissue biopsy
Sarcoid bone changes
terminal phalangeal bone cysts
punched out lesions in terminal phalanges on xray
Rx of stage 1 sarcoid
don’t need treatment as most recover spontaneously
