Interstitial lung disease Flashcards
Interstitial lung disease (ILD) def
Lung parenchyma diffusely affected
Chronic inflammation and progressive interstitial fibross
ILD sx
SOB on exertion
Non-productive cough
restrictive pulmonary spirometry
ILD causes
Environmental/occupation
GORD
Extrinsic allergic alveolitis
Associated with systemic illness
Idiopathic pulmonary fibrosis
Occupation/environmental causes of ILD
asbestosis
berylliosis
silicosis
cotton worker’s lung (byssinosis)
Systemic illnesses associated with ILD
sarcoid
rhuematic arthritis
SLE
UC
Extrinsic allergic alveolitis (EAA) def
hypersensitivy to certain allergens
EAA AKA
hypersensitivity pneumonitis
Causes of EAA
Bird-fancier’s lung
Farmer’s and mushroom worker’s lung
Malt worker’s lung
Bagassosis or sugar worker’s lung
EEA 4-6hr post exposure sx
Fever/ rigors
myalgia
dry cough
dyspnoea
crackles (no wheeze)
Chronic EAA sx
exertional dyspnoea
type I respiratory failure
cor pulmonale
CXR for EAA
Acute: Upper zone consolidation
Chronic: fibrosis, honeycombing
Acute EAA rx
Remove allergens
O2 35%-60%
Prednisolone
Chronic EAA rx
Avoid exposure to allergens
or wear a facemask or +ve pressure helmet
Long-term steroid
Idiopathich pulmonary fibrosis (IPF) def
a type of unknown cause interstitial pneumonia
commonest cause of interstitial lung disease
Sx of IPF
Dry cough
Exertional SOB
Wt loss
arthralgia
