What is the epidemiology?
Aged 20-39 or around 60
Women
Black ethnic origin
What is the presentation?
Dry cough
SOB
Erythema nodosum
Lupus permio
What is erythema nodosum?
Nodules of inflamed subcut fat on shins
Raised, red, tender, painful subcut nodules across both shins
What is lupus pernio?
Purple skin lesions on cheeks and nose
What organs are affected in sarcoidosis?
Lungs
Systemic
Liver
Eyes
Heart
Kidneys
CNS
PNS
Bones
What would be shown in blood tests?
Raised ACE
Raised calcium
what would be seen on X-ray for sarcoidosis?
Hilar lymphadenopathy
What would be shown on CT scan?
Show Hilar lymphadenopathy and pulmonary nodules
What would MRI show?
CNS involvement
What would PET scan show?
Active inflammation
What is the management of sarcoidosis?
Conservative management
Oral steroids + bisphosphonates protect against osteoporosis
Methotrexate
Lung transplant Fertility treatment
What is the prognosis of sarcoidosis?
Spontaneous resolve in half patients within 2 years
May progress to pulmonary fibrosis and pulmonary hypertension
