Definition of sarcoidosis
Multi-system disease characterised by granulomas formation, resulting in widespread inflammatory changes and complications
Acute symptoms of sarcoidosis
Fever, polyarthralgia, erythema nodusum, bilateral hilar lymphadenopathy
Most common chronic manifestations of sarcoidosis
Pulmonary - dry cough, dysopnoea, reduced exercise tolerance, crepitations
Constitutional features of sarcoidosis
Fatigue, weight loss, low-grade fever, lymphadenopathy, enlarged parotids
Neurological / ocular features of sarcoidosis
Meningitis, peripheral neuropathy, bilateral Bell’s palsy, uveitis, keratoconjunctivitis sicca
Other chronic features of sarcoidosis including the heart and abdomen
Arrhythmias, restricted cardiomyopathy, hepatomegaly, splenomegaly, renal stones, lupus permo
Skin feature in sarcoidosis
Erythema nodusum
Diagnostic test of sarcoidosis
Tissue biopsy of lung, lymph nodes finding non-caseating granulomas
Blood findings in sarcoidosis
Raised ESR, ACE and Ca2+, reduced lymphocytes
Stage 1 on CXR
Bilateral hilar lymphadenopathy
Stage 2 on CXR
BHL with peripheral infiltrates
Stage 3 on CXR
Peripheral infiltrates alone
Stage 4 on CXR
Pulmonary fibrosis
Management of BHL
Usually self-limiting
Management of acute symptoms
Bed rest and NSAIDs
