Definition of lung abscess
Liquefactive necrosis of lung parenchyma as a result of a pathogen infection, creating a cavity consisting of fluid and debris
Risk factors for lung abscess
Aspiration of oropharyngeal secretions, lung malignancy, pneumonia
Symptoms of lung abscess
Fever, productive foul smelling cough, dysopnoea, lethargy, night sweats, weight loss, cachexia
Signs of lung abscess
Clubbing, dental erosions, peridontal disease, localised dull percussion note, bronchial breathing
Investigations into lung abscess
Bloods,sputum culture, CXR and bronchoscopy
What is the most common signon CXR
Most commonly posterior segment of upper lobe and apex of lower lobe
Conservative management of lung abscess
Chest physio for postural drainage, smoking cessation
Medical management of lung abscess
Supportive of O2, fluid and analgesia, ABx IV for 3 weeks followed by oral Abx for 1-2 months
Surgical management of lung abscess
CT guided percutaneous drainage or surgical resection
