Pretty much Steroids
no real treatment but induce remission

stage 1 lung- No mx
Stage 2-4- oral Steroids, or steroids sparing therapies (mesalazine, azothiorine etc)–doesnt improve lung function much, just CXR

Cutaneous Sarcoid- topical steroids mainstay, occasional oral

Cardiac, neuro- higher doses, or combine with steroid sparing drugs
Manage complications of HF, etc

Non caseating granulomas in tissues in the body- can be anywhere but often lungs, heart, brain

afrocaribean > others
different ethinicities get in in different areas- eg japanse often heart sarcoid

Sx of sarcoid

Really vague
general- Tiredness, weight loss
But can be–
SOB progressive, dry cough, wheeze and ronchi
Joint Pains
HF signs
Lymphadenopathy
Erythema nodosum
swinging fevers with acute flares
Associated with uvueitis

Ix of Sarcoidosis

CXR-bilateral hillar adenopathy– (stage 1)
+ clouding/pulm infiltrates (stage 2-3-4)

restrictive lungfunction test (FEV1>FVC >0.7)

calcium raised- due to calcitriol over production by macrophages

FBC-Can be aneamic
