no sx / light vaginal bleeding, ↓pregnancy sx, closed cervix, nonviable fetus on US
missed SAB
vaginal bleeding, uterine cramps, +/- intrauterine fetus w/ heartbeat, open cervix
inevitable SAB
vaginal bleeding w/ passage of large clots/tissue, uterine cramps, products of conception often visualized in cervical os, open cervix
incomplete SAB
var vaginal bleeding, closed cervix, viable pregnancy on US
threatened SAB that can proceed to viable birth
fever, malaise, signs of sepsis, foul-smelling vaginal discharge, cervical motion & uterine tenderness, cervix usually open, usually retained products of conception on US
septic SAB
tx for threatened SAB
exptectant management until sx resolution or progression to inevitable/incomplete/missed abortion
tx for inevitable, incomplete or missed SAB
if hemodynamically unstable: surgical evacuation (D&C)
if hemodynamically stable: expectant management, prostaglandins or surgical evacuation
tx for septic SAB
blood & endometrial cx -> abx -> surgical evacuation of uterine contents
