How long do the effects of IVIG therapy last?

SCID, XLA, SVID, hyper IgM, IgG deficiencies
Side effects of IVIG therapy
Malaise, HA, fever, chills, anaphylaxis, dyspnea, uticaria, thrombosis, hemolysis, kidney injury
Why do we need to do patient blood work before administering IVIG?
To check renal and liver function, and also because the donor Ab’s will give artificial results reflective to the patient.
What are four hyper immune globulins
Rabies, Hep. B, Tetanus toxin, Varicella/Zoster
Why would we give a hyper immune globulin?
To “cover” the patient wrt Ab’s until the vaccine can take effect.
What kind of vaccine can’t we give with immune globulin therapy?
Live. Should be held for 6 months.
Hep. B immune globulin: when can we give it? How do we give it?
within 14 days of exposure.
It is administer IM (with IV available)
Rabies immune globulin: how do we give it?
Inject around the wound and then inject the rest IM in the gluteal region. One time dose.
Indications for tetanus immune globulin
uncertain of pt’s history of tetanus vaccination
OR
if they haven’t completed their tetanus series
AND
has a wound that is unclean or significant
How is tetanus immune globulin administered
IM and with vaccine
Varizig indications
post exposure (up to 4 days) prophylaxis for immunocompromised individuals without history of immunity; newborns of mothers with varicella or zoster within 5 days before or 48 hours after delivery.
Besides HDN, what are two other conditions treated by Rho(D)?
ITP and incompatible blood transfusions.
Describe Anti IgE therapy
This is called Xolair, aka omalizumab. MAB stands for monoclonal antibody. We treat moderate to severe uncontrolled asthma after treatments have failed and the patient has high levels of IgE (30-700). IT is a sub q injection every 2-4 weeks for life. VERY EXPENSIVE.
