Inhibits nucleic acid or protein synthesis

What form is Vidarabine?
Ointment 3%, only available in compounding pharmacies
How does Vidarabine attack virus?
Intervene with DNA synthesis. Has a purine nucleoside analog that resembles deoxydenosine.
What viruses do Vidarabine attack?
HSV-1 and 2, Varicella Zoster Virus.
What is the pharmacology of trifluridine?
It is a viroptic solution of 1%. It inhibits DNA synthesis. Thymidine analog. Originally used in cancer pts.
What is the clinical use of Trifluridine?
DOC HSV keratitis. 1gtt every 2 hours while awake. 6-7 days. Wait 5 mins after trifluridine to use lubricating drops so it is not diluted.
What are the side effects of Trifluridine?
Burning and stinging sensation, corneal punctuate keratpathy, severe dry eye (Keratoconjunctivitis sicca) and an icreased IOPs. Most prevalent in elderly.
How is virus treated?
With topical and oral medications.

more in imunno compromised pts. (HIV)
What is the pharmacology of Gancyclovir (Zirgan)
Will replace trifluridine. 0.15% Gel. Inhibits DNA synthesis.
What does Zirgan treat?
Herpes Keratitis. I gtt in eye 5 times a day. “Off label” more common in pediatric population.
what nerve does Herpes effect?
usually affects trigeminal nerve.
