List risks of ortho tx
Decalcification
Resorption
Recession
Relapse
Gingivitis
ST trauma - pain/ulceration
Describe relapse to a patient
When the teeth go back to where they were - back to being squint
Always a risk this might happen but the risk is higher if you don’t comply with the instructions given about wearing a retainer at night after ortho tx is completed
What cases are more prone to relapse?
Class II Div II
Crowded lower anteriors
Describe resorption to a patient
When the roots of teeth become shorter after tx
Some resorption is inevitable - due to nature of Otho tx moving the roots of teeth ,1mm is normal
However if more than this is noted may need to stop tx / require further
Reassure that we monitor this with X-rays
What cases are more prone to resorption?
Prev trauma
Blunt roots prior
Pts with habits: nail biting
Describe decalcification to a patient
When white spots appear on teeth, which is a sign of early reversible tooth decay
This is caused by poor OH + diet
Really important to practice good OH esp with ortho as it makes it harder to clean between teeth due to the brackets
Brush 2x daily, after every meal - spit dont rinse
Target tricky areas around brackets + gum
Use tools: single tufted, superfloss
Reduce sugar amount + frequency - avoid snacks between meals, sugary drinks
Describe recession to a patient
When the gums shrink back and we can see more root than usual
Aesthetic
Root caries (root exposed less mineralised easier for decay to form)
Sensitivity
Describe braces to a patient
Train tracks, metal squares glued onto teeth fixed option
How do we manage relapse?
Removable retainers
Fixed retainers
Options for removable retainers
Thermoplastic
(clear, vacuum, Essex,hawley)
COMPLIANCE DEPENDENT
Options for fixed retainers
Fixed wire bonded on with composite
DEBOND CARIES + ST TRAUMA
