Components of CHA2DS2-VASc in a history?
- Intro - AGE and SEX
- PMHx - HF, HTN, DM, previous stroke/TIA, previous MI
What is Well’s score?
For DVT/PE
What are the components of Well’s?
- SOCRATES - entire leg swollen, >3cm than other leg, pitting oedema, tenderness, collateral superficial veins
- PMHx - Hx of DVT, active cancer, paralysis/recent plaster, bedridden >3 days, surgery in past 12 weeks
- Social Hx - recent long haul travel
What is socring for CHA2DS2-VASc?
1+ M and 2+ F = anticoagulant
What is scoring for Well’s?
0 = low (D-dimer)
1-2 = moderate (D-dimer)
2+ = high (D-dimer and doppler)
+ve = LMWH
What is used for Diabetes?
NICE guidelines
Components of Diabetes guidelines in history?
- Identify - Age, Sex, Ethnicity
- PMHx - HTN
- FHx - diabetes
- Closing - BMI/waist circumference
Scoring for diabetes guidelines?
0-6 = low (lifestyle modifactions)
7-16 - increased (changes eg. diet, exercise)
16-24 = moderate (changes and bloods)
25+ = high (changes and bloods immediately)
What is QRISK?
MI/CVA risk in next 10 years
What are components of QRISK in history?
- Identify - age (25-84), sex, ethnicity
- PMHx - migraines, mental health, AF, DM, CKD stage 3, erectile dysfunction, SLE, RA
- Drug Hx - antihypertensives, steroids, antipsychotics
- FHx - MI or angina
- Social Hx - smoking
- Closing - BMI, BP, HDL ratio
Scoring for QRISK?
if >10%, consider statin
What is FRAX?
Risk of major/NOF fracture
Components of FRAX in history?
- Identify - age and sex
- PMHx - fracture HX, RA, secondary osteoporosis
- Drug Hx - steroids
- Family Hx - parent with NOF fracture
- Social Hx - smoker, alcohol >3 units/day
- Closing - height, weight, bone mineral density
Scoring for FRAX?
<10% low risk (lifestyle changes). >10% high risk (DEXA and bisphosphonates)
