Why must a risk assessment be completed?
To assess what employees do and how to mitigate the risks they are exposed to
What is an accident?
An unplanned, undesired occurrence which may result in harm or loss
What is a hazard?
Anything that has the potential to cause loss or harm
What is a risk?
How likely it is that a hazard will actually cause harm or loss
What are the stages of risk assessment?
HAZID
HAZOP
Evaluation of risks
Implementing control measures
Recording findings
Reviewing & revising
What is HAZID?
HAZard IDentification
The first stage in risk analysis identifying potential causes of harm from equipment, electricity, substances and environment
What is HAZOP?
A structured and systematic technique for risk management that identifies potential hazards
How are likelihood and consequence measured?
In tables from 1-5, 1 being rare/insignificant, 5 being almost certain/catastrophic
What does a risk matrix do?
Defines level of risk considering probability of an incident
The four risk levels are low (green), medium-low (yellow), medium-high (orange) and high (red)
What are different controls of risk?
Elimination (i.e. removal of dangerous chemicals)
Reduction/substitution (i.e. using a less flammable substance)
Isolation (i.e. operating a machine remotely)
Engineering controls (i.e. machine guards)
Administration controls (i.e. different training)

What are some typical control measures?
Machine guarding
Machine isolation
