Dangerous occurrence
A
Unplanned occurrence which has the potential to cause injury.
(Near miss that could lead to serious injury or loss of life)
- submit report within 10 days
- Overturning or failure of lifting equipment
- failure of a pressure system
- fire causing stoppage of plant for over 24hrs
- partial/complete collapse of scaffold
- incident when contact made with power lines over 200 volts
