What is rhinosinusitis?
Inflammation of the nose and paranasal sinuses.
What must a patient have to be diagnosed with rhinosinusitis?
Two or more symptoms of:
- Nasal blockage/obstruction
- Congestion/nasal discharge
- Facial pain/pressure
- Reduction/loss of smell
What are the types of allergic rhinosinusitis?
- Seasonal
- Perennial - throughout the year
- Occupational
What is this a presentation of?
Sneezing, pruritus, clear nasal discharge, bilateral red itchy eyes.
Allergic rhinosinusitis
What is the treatment for allergic rhinosinusitis?
- Allergen/irritant avoidance
- Nasal saline irrigation
- Loratadine 10mg OD
- Intranasal corticosteroid sprays
What is this a presentation of?
Middle aged male with bilateral watery rhinorrhoea, sneezing, purulent postnasal drip, nasal obstruction, mouth breathing, and snoring.
Chronic rhinosinusitis with nasal polyps
What conditions is chronic rhinosinusitis with nasal polyps associated with?
Allergic rhinitis, cystic fibrosis, aspirin hypersensitivity, late onset asthma.
What is the management for chronic rhinosinusitis with polyps?
- Refer to ENT for anterior rhinoscopy/nasal endoscopy (urgent if unilateral)
- Topical steroid drops for 2 weeks followed by fluticasone for 3 months.
- Endoscopic sinus surgery if refractory to maximal medical management.
What is the management for chronic rhinosinusitis without nasal polyps?
Nasal irrigation and intranasal steroids
