What is this a presentation of?
Discoloured discharge from nose, severe facial pain (worse when bending forward), fever, worsening after initial milder illness.
Bacterial acute rhinosinusitis
What is the management of acute rhinosinusitis?
- 98% viral, self-limiting, 2.5 weeks to resolve
- Simple analgesia, nasal saline irrigation, intranasal decongestions.
- Bacterial suspected/>5 days present - amoxicillin
What is the main complication of acute rhinosinusitis?
Periorbital cellulitis - spread of infection from ethmoidal air cells to periorbital cavity, admit, IV Abx
What are the intracranial complications of acute rhinosinusitis?
Meningitis, cerebral abscess, osteomyelitis.
