What criteria do you need for a diagnosis of rheumatic fever?
- 2 major
- 1 major, 2 minor
PLUS evidence of preceding GAS
What are the major criteria for rheumatic heart disease (+any changes if high risk)? (5)
- Evidence carditis (high risk: can be subclinical valvulitis seen on echo)
- Polyarthritis (HR: can be polyarthralgia or monoarthritis)
- Subcutaneous nodules
- Chorea
- Erythema marginatum
What are the minor criteria for acute rheumatic fever?
- Prolonged PR interval
- Polyarthralgia or monoarthritis (this is a major for HR. Minor for HR is monoarthralgia)
- CRP >30, ESR >30
- Fever
What is the nature of arthritis in ARF? (number joints, how it changes, response to analgesia)
- Polyarthritis
- Migratory (can leave a joint after 1 hour even) and additive
- Responds to NSAIDs
What valve is most affected in rheumatic heart disease?
Mitral valve (50-60%) (MR + aortic is 20%)

TNF-alpha
