Mx of RA?

A

Initial Mx: DMARD mono therapy +/- bridging with a short course of Prednisolone.

Options:
Methotrexate (FBC & LFTs monitoring as myelosuopression & liver cirrhosis risk)
Sulfasalazine, Leflunomide, Hydroxychloroquine

Monitoring response to Rx:
CRP + DAS28 score to monitor disease activity.

RA flare: PO or IM corticosteroids e.g. Methylprednisolone

If inadequate response to 2 DMARDs (inc. Methotrexate) = Biological Rx (TNF-inhibitors)

E.g. infliximab, etanercept, adalimumab, Rituximab, Abatacept
