Rheumatic Fever DX
Jones criteria:
positive
for rheumatic fever when either 2 of the major criteria or 1 major criterion
plus 2 minor criteria are present, along with evidence of streptococcal infection
(i.e., elevated or rising antistreptolysin O titer or DNase).
Jones Major criteria
- Migratory polyarthritis
- Carditis (myocarditis, pericarditis)
- Erythema marginatum
- Subcutaneous nodules
- Chorea
Jones Minor criteria
- Fever
- Antecedent strep infection
- Arthralgias
- Elevated ESR (vsg)
- Prolonged PR internal
- Heart block on ECF
Rheumatic Fever Tx
antibiotics to eradicate group A streptococcal
bacteria.
NSAIDs or steroids.
if mitral valve disease: chronic penicillin therapy
Ebstein Anomaly Path
maternal lithium use in pregnancy> tricuspid valve is
downwardly displaced into the right ventricle.
Ebstein Anomaly Path Cx Ft
Physical examination will show a
holosystolic murmur of tricuspid regurgitation over most of anterior left chest.
Ebstein Anomaly Path Dx
EKG will show tall P waves and right axis deviation
+- Wolff-Parkinson-White
syndrome (delta wave and short PR
interval).
Vascular Ring Cx Fx
biphasic stridor or dysphagia with spitting up
after meals from compression.
respiratory symptoms that improve with neck extension
parents that their child is a “noisy breather
Vascular Ring is frequent in
DiGeorge syndrome or
Down syndrome
Vascular Ring Dx
CT or MRI.
Vascular Ring Tx
symptomatic vascular
rings should undergo surgical correction. Asymptomatic, incidentally found
rings should be monitored.
