A

Skeletal muscle breakdown causing release of K+, PO4, urate, Myoglobin, CK
Hyperkalaemia

What can cause Rhabdomyolysis?

A

Ischaemia - Embolism, Surgery
Trauma - Immobilisation, Crush, Burns, Seizures, Compartment syndrome
Toxins - Statins, Ecstasy, Fibrates

What is the clinical presentation of Rhabdomyolysis?

A

Muscle pain
Swelling
Red/Brown urine
AKI follows 12 hours later

A

Dipstick: +ve Hb, -ve RBCs
Blood: ↑CK, ↑K, ↑PO4, ↑urate

What is the management of Rhabdomyolysis?

A
Manage hyperkalaemia
Fluids
Monitor CVP is oligouric
IV NaHCO3 may be used to alkalinize urine and stabilise
a less toxic form of myoglobin.
