Revision Complete Flashcards
What should be recorded on a primary impression for complete dentures?
Record all denture bearing areas, rolled sulcus, extended far enough posteriorly
-> Consider modifying stock trays with compound and greenstick
What are finger rests good for?
Good if resorbed ridge, helps stabilise tray
What are stub handles helpful for?
Border moulding
What materials are used in close fitting trays?
ZOE
Light bodied silicone
What are close fitting trays good for?
Resorbed ridges
If you have no previous guide for mould what can help the technician ascertain?
Draw canine lines
What are the uses of cuspless teeth?
Bruxism-allows excursive movements
Very resorbed lower ridge
-> displacement on excursions and occlusal interferences will not displace denture
What curves are important when setting teeth?
Curve of spee- anterior posterior curve
Curve of Monson- coronal curve
-> Even bilateral contacts on excursive movements (in natural teeth we have canine guidance which would displace denture)
What should be checked at try in stage in complete dentures?
Vertical dimension of occlusion
Even contact in retruded contact position
Lip support
Occlusal planes – anterior/posterior
Lower teeth over ridge
Speech
Retention & stability
Base extensions
Appearance
What are the issues with try in stage?
Difficult to assess retention
What are the steps in processing a try in to produce a denture?
Wax up
Flasking- put dentures in flask with plaster and melt out wax and replace with acrylic
Deflasking
Trim & polish denture
What does a post dam do?
Creates posterior seal
-> compresses tissues to allow maintenance of seal
What is checked on insertion of finished complete denture?
Vertical dimension of occlusion
Even contact in retruded contact position
Speech
Retention & stability
Base extensions
Appearance
Comfort
How is extension checked?
Extensions- pull on cheeks and lips
Lateral movement of bottom jaw- shows overextension of the upper denture at the coronoid process (may need to adjust polished surface)
What must labs producing prostheses be registered with and what law does this pertain to?
Labs must be registered with MHRA
Medical device regulations 2019
What is checked at review of complete dentures?
- Pain redness ulceration- pressure indicating paste
- Function
- Aesthetics
- Speech
- Recheck occlusion
- OVD (difficult to change)
When are replica dentures indicate?
if patient well adapted to dentures and occlusion has changed very slightly/teeth have worn
What are the clinical stages in complete denture treatment
How are teeth set for complete dentures
What are the ADV/DIS of replica technique?
ADV
Keeps existing design
Easier adaption
Maintains existing aesthetics
1 less appointment
Only 1 impression
Jaw reg is simpler
Easier to observe changes to original
DIS
Major modifications difficult
Can reproduce faults
What are the advantages/disadvantages of conventional technique for complete dentures?
ADV:
Can be used if no previous denture, previous was unsuccessful
Can change aesthetic significantly
Major modifications and faults can be corrected easily
Good if significant resorption since original dentures made
DIS:
Takes longer
Complex jaw reg
2 impression stages required
