Who needs Statin therapy?

A

Hx of ASCVD (secondary prevention or risk equivalent)
* < 75 years old: High intensity statin
* > 75 years old: moderate intensity statin (sometimes will see simvastatin 20-40mg)
* target LDL decrease by 50%, very high risk? goal < 70

LDL > 190mg/dL (primary prevention)
* High intensity statin: atorvastatin 40-80mg, rosuvastatin 20-40mg

Diabetes (ages 40-75)
* moderate intensity statin
* ASCVD risk > 7.5% high intensity

Pt w/o diabetes or ASCVD (ages 40-75) with estimated 10 year ASCVD risk > 7.5%
* moderate to high intensity statin

  • ABI first line claudication
  • sign of occlusive dz, usually ultrasound
  • other imaging only after ABI or US (CT, MRI, cath)

what labs should be done for initial evaluation of HTN?

A
  • ECG: screening for LVH
  • creatinine, UA: screening for kidney dysfuntion
  • TSH: Screening for secondary cause
  • Glucose/A1C: Assessing for other CV risk factor
  • Lipids: Asessing other CV risk factor
