What are the 3 types of retinal detachment?
Rhegmatogenous —> tear/hole in neurovascular layer —> vitreous humour can leak through & push th neuro layer from the pigmented epithelium
- most common
- tears occur trauma, iatrogenically, degeneration
Tractional—> vitreous humour has abnormal attachment to retina
- occurs in diabetic retinopathy, retinopathy of prematurity, retinal vein occlusion
Exudative —> collection of fluid but no tear ~ usually from a infectious or inflammatory process (scleritis)
What is retinal detachment?
Detachment of the neural layer of the retina from the pigmented epithelium
What are S&S of it?
EMERGENCY
Painless
Flashing lights
Floaters ~ stuck in line of vision with eye movement
Loss peripheral vision (black curtains closing)
Loss of central vision
Sudden in onset
Risk factors?
Head or eye trauma
fam hx or personal hx of RD
Increasing age
Myopia
Diagnosis?
Fundoscopy ~ grey/white colour, loss of red reflex
Prognosis & treatment?
Depends on cause - treatment: laser can close holes/tears
Emergency if macula is still attached to pigmented epithelium
