Restoring the Carious Primary Dentition Flashcards
Why restore primary teeth?
Avoid pain Avoid sepsis Avoid extraction Restore form Restore function Maintain space for permanent dentition Restore aesthetics
Impacts of pain?
Difficulty eating Loss of sleep (tiredness at school) Time off school Problems with toothbrushing Interference with social activities
Impacts of infection (sepsis)?
Severe early childhood caries = failure to thrive
Preschool children with severe early caries = weigh less and are smaller than peers
6x more likely to have iron deficiency anaemia than peers
GA prevalence?
58,000 GA for caries
30 million
25% increase in 10 yrs
How do restored teeth, fluoridated areas effect the need for extraction?
Filled teeth less likely to be extracted than unfilled teeth = FILLING’S WORK
Teeth in fluoridated areas survive longer than those from non-fluoridated areas
Effect of tx is greater than the effect of fluoridation status
Barriers to treating young children’s teeth?
Pre-cooperative children Pt anxiety - fear of unknown Skill/attitudes of the dental professional Parent view of materials Funding
Survival rates of restorative materials used in primary teeth?
PMC 96% RMGIC 93% Compomer 91% Amalgam 82% Composite 73% MRMGI 57%
Rubber dam more likely to survive Class I's more likely to survive than class IIs
Who to not use amalgam in?
What is the exception?
Pregnant or breastfeeding women
Children under 15yrs
Deciduous teeth
EXCEPT when necessary by the dental practitioner based on the specific medical needs of the pt
Need to document if using amalgam
When are PMCs used?
Large multi-surface restorations - more than a 3rd of the buccal-lingual width After pulp therapy = creates a seal Fractures Dental anomalies Space maintainer
How does LA impact restorations?
Restorations with LA = more successful
Advantages of the hall technique?
Child friendly Quick Effective Cost effective With the hall technique - do NOT remove the caries, NO LA
What restorative materials can be used in children?
GIC
Composite
PMC
Atraumatic restorative technique
Used when no access to electricity
Complete caries removal
Glass ionomer over it
What does carisolv do?
Put it in the cavity - will remove carious dentine
BUT a cavity is needed
Non-restorative caries tx?
Try to maintain contact
If necessary make a chamfer prep
F varnish and OHI
Hall technique works better
