cough (productive or not)
dyspnea/SOB
orthopnea (trouble breathing in supine position)
chest/rib/muscle pain
Document respirations?
Rate: bradypnea, tachypnea, eupnea
Rhythm: regular, irregular
Volume: shallow, deep, hypo/hyperventilation
Sounds: crackles, ronchi, wheezing, stridor
Assessment of common Respiratory Symptoms?

Vertebrae?
C 1-7 T 1-12 L 1-5 Sacrum Coccyx
Adventitious sounds?

What to observe for when pt is breathing?
accessory muscle use (skin btwn ribs in concave on inhalation)
rise and fall of chest
use of shoulder muscles to breathe
Palpation?
trachea
supersternal notch
NO PERCUSSION
Check for good air entry bilaterally
