Pathophysiology type I
- inadequate gas exchange
- hypoxia
-paO2 <8kpa - primary cause is V/ Q mismatch
What can cause v/q mismatch
pneumonia, PE, pulmonary odema, asthma
Investigations for resp failure
FBC, u&e, ABG, CRP
CXR
sputum and blood culture
spirometry
Management of type 1 resp failure
O2 20-40% by facemask
Assisted ventilation
History for resp failure
hypoxia- confusion, agitation, cyanosis, dyspnoea
hypercapnia- headache, coma , flap, tachy, bounding pulse
Type II resp failure pathophysiology
Hypoxia <8kpa + hypercapnia >6 kpa
caused by alveolar hypoventilation
What causes alevolar hypoventilation
asthma, copd, pneumonia, CNS tumour, trauma, GB, myasthenia gravis
Management of type II resp failure
24% O2 the check ABG if CO2 has gone down increase o2 to 28% if not then look at assisted ventilation
