Dyspnea (hallmark)
Cough
Chronic sputum
Wheezing
For the medical history , what do you want to ask /know ?
Exposure to risk factors
-smoking
-occupational and environmental exposures to other lung irritants
How you calculate the total pack years ?
Number of cigarettes smoked per day divided 20 times number of years of smoking
So what investigation you want to do to diagnose this case ?
Spriometry
Bronchodilator reversibility testing
Chest radiograph
On chest radiograph , what do you expected ?
Hyperinflation
Bullae
Hyperlucency of lungs
What is the DDx of COPD ?
Bronchial asthma
Bronchiectasis
Pulmonary TB
Drug used in mild acute COPD attack
SABA
or
SAAC
or
Both
Drug used in severe acute COPD attack
LAAC
or
LABA
or
ICS+ LABA
ICS - inhaled corticosteroid
If the patient are symptomatic despite maximum inhaled therapy , what will you give ?
Theophylline
Theophylline is bronchodilator
What does grade 0 MMRC means
not troubled breathlessness except on strenuous exercise
What does Grade 1on MMRC means ?
Shortness of breath when hurrying on the level or walking up a slight hill
What does Grade 2 on MMRC means
Walks slower than the same age on the level because need to stop for breath
What does Grade 3 on MMRC means ?
Stops to breath after walking about 100m
What does Grade 4 on MMRC means
too breathless to leave the house
or
breathless when dressing or undressing
