Lung transplant indications
A
Age - unilateral <65y, bilateral <60y, heart/lung <55y
COPD - FEV1<25%, CO2 >55mmHg
CF/bronchiectasis - FEV1<30%, severe complications, CO2>50, O2<55
ILF - Sx, DLCO<60%
Pul HTN - NYHA 3/4, PAP>55, CO <2L/min
Eisenmengers - prognosis not improved

Lung transplant contraindications
A
Absolute - HIV, hepB, hepC cirrhosis, malignancy, organ failure
Relative - diabetes, OP, alcohol, smoking, nonadherence, MAC, weight >130% or <70% ideal

A
2 months RIPE, 4 months RI
Monitor sputum cultures -> negative
MDRTB - streptomycin, quinolone, amikacin, cycloserine, ethionamide, PAS
