Fe Asthma
Expiratory effort = lower airway obst
if fast, hard to tell phase
wheezes on expiration due to narrowed airways
+/- Paradoxical breathing
bact pneumonia
Mixed inspiratory and expiratory effort = pulmonary parenchymal dis
Incr BV sounds = wet lungs
crackles if alv full of pus
wheezes if airways narrowed d/t mucus
pyothorax
Shallow, inspiratory effort
No noises w/o stethoscope
Muffled heart and lung sounds ventrally
No adventitial lung sounds unless also pneumonia
Paradoxical breathing common
Hyporeasonant on percussion
idiopathic pul fibrosis
Shallow breaths inspiratory restricted lung dis
Harsh crackles
Pulmonary hypertension may lead to cor pulmonale with tricuspid regurgitaiton
pneumothorax
Shallow, rapid inspiratory effort
+/- Muffled lung and heart sounds
Hyper-reasonant on percussion
Depending on cause, no adventitial sounds
cardio pul edema
Heart murmur +/- arrhythmia
Mixed inspiratory and expiratory effort = parenchymal disease
Increased BV sounds = wet lungs
Crackles if alveoli full of water, esp after cough
Often orthopnic
laryngeal paralysis
Voice change/weak bark
“Horsey” breathing
Overt stridor
