Resp Meds Flashcards
name of Beta2 adrenergic agonists
“terol”
- albuterol
- formoterol
- salmeterol
- terbutaline
pharma action of beta2 adrenergic agonists
activating beta2 receptors in bronchial smooth muscle
resulting in bronchodilation
3 effects of beta2 adrenergic agonists
- relieve bronchospasm
- histamine inhibited
- ciliary motility increased
therapeutic uses of beta2 adrenergic agonists
albuterol for both short & long acting asthma
formoterol, salmeterol, terbutaline long acting asthma
prevent exercise induced bronchospasm
side effects of beta2 adrenergic agonists
tachycardia, angina -> check pulse increase of >20 (lower dose)
tremors -> may resolve or lower dose
nervousness, restlessness
contraindications/side effects of beta2 adrenergic agonists
- contraindicated in pts w/ tachydysrhythmia
interactions with beta2 adrenergic agonists
- beta adrenergic blockers make meds useless
- tricyclic antidepressants, MAOIs increase risk of tachycardia & angina
- antidiabetic drugs need to increase dose
- no caffeine
names of Methylxanthines
theophylline
pharma action of Methylxanthines
- relax bronchial smooth muscle -> bronchodilation
therapeutic use of theophylline
oral used for long-term control of asthma
IV for emergencies
side effects of theophylline
mild: GI distress (V/D), restlessness/insomnia
severe: dysrhythmias, seizures
therapeutic range of theophylline
5-15 mcg/mL
treating toxic effects of theophylline
- activated charcoal to reduce absorption
- diazepam for seizures
- lidocaine for dysrhythmias
contraindications/precautions of theophylline
- impaired metabolism
- tobacco/marijuana use
- caffeine
interactions with theophylline
- caffeine
- phenobarbital, phenytoin, rifampin (seizure meds) increase metabolism
- cimetidine, ciprofloxacin (fluoroquinolones) increase risk for toxicity
name of inhaled anticholinergics
ipratropium
tiotropium
“tropium”
pharma action of inhaled anticholinergics
inhibits acetylcholine in parasympathetic system -> bronchodilation
therapeutic uses of inhaled anticholinergics
- relieve bronchospasm of: COPD, allergen-induced, exercise-induced
- decrease secretions in COPD
side effects of inhaled anticholinergics
- dry mouth, hoarseness -> drink water, suck on candies
- increased intraocular pressure
- urinary retention
contraindications/precautions with inhaled anticholinergics
- contraindicated for pts with peanut allergies
nursing teachings for pts using inhaled anticholinergics
- wait 5 mins between inhaled doses
- don’t swallow capsules, use inhalation device
names of glucocorticoids
- inhalation: beclomethasone (QVAR), fluticasone (Flonase)
- oral: prednisone
pharma action of glucocorticoids
- prevent inflammation, suppress airway mucus production, promote responsiveness to beta2 receptors (use albuterol before glucocorticoid)
- decrease airway mucosa edema
therapeutic use of glucocorticoids
- long term chronic asthma - inhaled
- short term post exacerbation
- take albuterol before glucocorticoids
