The health facility must place a copy of transfer/discharge notice to:

resident can pay balance owed to facility up to the date of transfer/discharge and remain in facility
intrafacility transfers can be made only if:
medical reasons as judged by physician OR transfer is necessary for welfare of resident or other persons

safety or health of individuals in facility is endangered
resident’s health improves sufficiently
resident has urgent medical needs
written notice of intrafacility transfer must include:
