Research the medication on AUSDI
Before we start, I’ll just research the medication on AUSDI, checking the following:
- Drug name
- Indication
- Dose (must match the dose prescribed by the doctor)
- Method
- Contraindications
- Precautions
- Adverse effects
Research the medication on the Injectable Handbook
I’ll also refer to the Injectable Handbook, verifying:
- Drug name
- Dose
- Preparation (pre-prepared or not)
- Administration method
- Special notes
After researching the medication..
- Let the patient know about potential adverse effects, such as nausea and drowsiness.
- “I’ll inform the patient about the adverse effects in a moment.”
- Ask the patient: “Are you still okay with us administering this medication?”
Gathering the equipment
- Perform hand hygiene
- State that you have clean the kidney dish and the trolley prior to starting.
- Gather all the equipment (it should already be provided to you during the cse)
1st check of the medication
Check the expiry date on:
- 3ml syringe
- Alcohol swab
- Drawing-up needles
- 25mm blue needle
- Label and wipes
- Ampoule
This is where I will conduct the first medication check:
- Drug name
- Dose
- Expiry date
Drug Calculation on Syringe Label
- Hand hygiene
- Include the following:
- Route
- Patient ID and date of birth
- Medication
- Amount (units) / Volume (mL) = Concentration (units/mL)
- Don’t sign the label as this will be done after the 2nd medication check
Right calculation for the label
Front - Ondansetron 4mg/2ml (2mg/ml)
Back - 4mg/4mg X 2ml = 2ml
