Addressing the patient (3rd check)
- Hand hhygiene
- “Hello Mr. Black, I have your medication ready, but I’ll be doing a final check.”
- Verify the patient name, dod, URN, date and time, generic drug name, route, dose, frequency, and time.
- Check allergies and allergy band.
- Close the curtains for privacy.
- Place the sharps container next to you for proper disposal.
Locating the site

“I will now be locating the ventrogluteal site where I will be putting the injection.” - The heel of the hand on the greater trochanter.
- Thumb pointing toward the person’s groin.
- Point index finger to the anterior superior iliac spine
- Stretch middle finger toward on the iliac crest
- Area between two fingers, about knuckle length, is the injection site
Cleaning the site
Wipe the site with an alcohol wipe in a circular motion (5cm) and allow to dry.
Perform and explain the Z track method
- In order to do the injection, I will need to perform the Z track technique:
- The Z track technique is stretching the skin before administering the medication.
- This is commonly utilised to prevent medication leakage.
Aseptic Hand Wash
Aseptic hand wash for 30-60 seconds and put gloves on
How to inject the needle
- 90 degree angle, dart like motion into the muscle (to the needle hub)
- Aspirate - Draw back on syringe to check the needle is not in a blood vessel
- Inject 1 mL/10 seconds, wait 10 seconds before withdrawing needle
Address the patient
“Ok Jack all done, are you comfortable in that position, here’s your call bell let me know if you need anything”
Documentation
“After I have administered the medication, I will be completing the proper documentation such as signing the documents that I have given the patient the proper medication, dose at the appropriate time”
Discard sharps safely
Make sure to put the sharps correctly at the sharps container
