Pregnancy outcome
Miscarriage rate similar to normal population
95% pregnancies are successful
Congenital anomalies similar to general population
Ectopic is higher due to adhesions from previous surgery and peritoneal dialysis
Hypertension is common and therefore PET is a risk and UTI in 40%
Also GDM
PTL, IUGR, PPROM in 60%
Rubella, CMV, toxo, Hep b/c and herpes are more likely to affect the fetus should mother catch it
Incidence
12% and increasing
Timing of pregnancy
There should have been no rejection in the previous year
Graft should be stable and adequate
No or mild proteinuria (< 15mg/day, Aziathoprine <2 mg/ day
There should be no acute infection
Co- morbidities should be assessed and managed
Meds
Safe: Prednisolone Aziathoprine Ciclosporin Tracolimus
Unsafe: MMF, risk of malformations and I trimester miscarriage Rituximab Sirolimus Everolimus
Neonatal complications
Thymic atrophy Transient leukopenia Thrombocytopenia Adrenocortical insufficiency Septicaemia Infections
