What is matched between patient and donor in kidney transplant
HLA type A,B,C on chromosome 6
They dont have to fully match by - weaker matches are more likely to get rejected
Treatment is given to disensitise them to donor HLA when there is a living donor
Typical scar found in kidney transplant:
hockey stick scar/incision - anterior abdomen
Where is implanted kidney palpable
in the iliac fossa area
Anastamoses made between implanted kidney and patient:
Pelvic vessels - usually external iliac vessels
Ureter of donor kidney anastamosed directly with bladder
immunosuppression regime classically given post transplant: (x3)
Tacrolimus
Mycophenolate
Prednisolone
Other possible immunosuppressants:
Cyclosporine
Sirolimus
Azathioprine
Post transplant complications (relating to transplant)
- Transplant rejection (Hyperaccute, acute, chronic)
- Transplant failure
- Electrolyte imbalances
Post transplant complications (relating to immunosupression)
Ischaemic heart disease
Type 2 diabetes (steroids)
Infections are more likely and more severe
Unusual infections can occur (PCP, CMV, PJP and TB)
Non-Hodgkin lymphoma
Skin cancer (particularly squamous cell carcinoma)
