4) Is the patient receiving renal replacement therapy (RRT), if so how often and what type?

• What is the indication for the drug and have alternatives been considered?
• What drug would normally be used if the pt did not have renal impairment?
• Is the patient currently taking the drug in question?
• What is the pt eGFR and CrCl?
