A
eGFR < 60ml/min/1.73m2 for >/= 3 months Or Evidence of kidney damage >/= 3 months Albuminuria Haematuria structural abnormalities Pathological abnormalities

Diagnostic evaluation for CKD
A
Renal US Repeat EUC, egfr, albumin FBC, CRP, ESR Urine ACR Fasting lipids and glucose Urine MCS

EGFR may be unreliable in
A
Acute kidney injury On dialysis Recently ate cooked meat Exceptional dietary intake Extremes of body sizes <18 yo Severe liver disease Drugs (e.g. fenofibrate or trimethoprim) Pregnancy

Increase urine albumin excretion
A
UTI High protein diet CCF Acute febrile illness Heavy exercise within 24hr Menstruation or PV discharge Drugs (e.g. NSAIDs)

Indications to referral to nephrologist
A
EGFR < 30mL/min/1.73mm2
Persistent significant albumonuria (ACR >/= 30mg/mmol)
A sustained decrease in EGFR >/= 25% Or EGFR 15ml/min/1.73mm2 within 12 months
CKD with HTN hard to get to target despite at least on 3 agents
