What is a refill?
- Filling the Rx amount after the inital fill has been dispensed
What is the refill requirements for C-IIs in Indiana?
- Within ONE year from date written & CANNOT be refilled
NO LIMIT TO HOW MANY A C-II CAN BE WRITTEN FOR
Since C-IIs have NO refills, what is one thing that the doctor can do?
- 90-day supply at one time [writing multiple Rxs]
When doing a 90-supply for C-IIs, what are some of the requirements to be able to do that?
- Rx MUST have legit purpose
- MUST have issued date & earliest to fill
- Dr says not risk of abuse
- Action must be legal in that state
- MUST meet C-II requirements
PROHIBITS post-dating; should have “2/21/24 & DO NOT DISPENSE BEFORE 3/21/24”
What is the refill requirements for C-III, IV, V?
- Within SIX months from issue date & have FIVE refills
What refill information should be kept for controls and how should it be kept?
- Written on the back of the Rx OR Computer system
- Name/Dosage of Control, Date filled/refilled, quantity, initials, total refills
What are the two recordkeeping methods for refills?
- Print out control refills
- Bound logbook that is filled out
What is a partial fill?
- Filling that is less than what is prescribed
Helps patient see if they like it, cant afford, not enough stock
What are some of the rules behind partially filling a C-II?
- Can if NOT prohibited by state law
- Patient, Patients Representative, or Doctor okays it
- Partial fill isnt more than total quantity
- Remaining gets filled within 30 days
- Doesnt apply in emergencies [72h]
Coming from the patient, represetative, or doctors means if you dont have enough; it follows the 72h rule and you have to fill the rest in 30d
How much are you able to fill when the prescriber write the amount they want?
- MUST specify the amount on the face - we ONLY fill that amount.
What are some of the situtations where a partial filling is ok for C-II products?
- Requested by Patient, Representative, or Doctor; partially fill UP TO 30 days
- Patient is LTCF; partailly fill UP TO 60 days, MUST not “LTCF patient or Terminall Illness”
- Other than these two; can partial fill BUT have to fill remainder within 72h
What are some of the rules behind partially filling C-III, IV, V?
- As long as dispensed doesnt exceed total
- NOT after expiration
- CAN fill more than 5 fives
What are some of the emergency situtations where a C-II can be given over the phone?
- IMMEDIATE administration for proper treatment
- NO other alternatives
- NOT possible to get written Rx before dispensing?
ALL must be met before taking a C-II over the phone
How much should an emergency prescription be?
- JUST ENOUGH for the “emergency period”
What are some of the important rules/laws that go with emergency prescriptions?
DEA??
- Within 7 days the doctors needs to send a “covering prescription”
- NO Covering: Pharmacist MUST tell DEA [would make it void - also if dispensed = misbranded]
- Central Pharmacies CANNOT fill emergencies
Covering Prescription must say Authorization fro Emergency Dispensing
How can a C-II be filled at a Central Fill Pharmacy?
- Pharmacist MUST write CENTRAL FILL on the OG paper Rx
- The Rx must be complete because Central Fill Pharmacies CANNOT change the Rx
What are some of the things that need to be on a C-II label?
- Date of Filling
- Dispensing Pharmacy Name/Address
- Rx Number
- Patient Name
- Prescriber Name
- Directions
- Cautions, IF ANY
Label doesnt apply to C-II when IN an institution as long as its no more than 7 DAYS
What are some things that need to be on a C-II label from a Central Fill Pharmacy?
- Same a noraml C-II label +
- Pharmacy Name/Address
- Unique Identifier [DEA Number]
What are some of the things that need to be on a C-III, IV, V Label?
- Date of Filling
- Dispensing Pharmacy Name/Address
- Rx Number
- Patient Name
- Prescriber Name
- Directions
- Cautions, IF ANY
Label doesnt apply to C-III, IV, V when IN an institution as long as its no more than 34 DAYS or no more than 100 units dispensed at once
What is the wording on the “No Transfer” Label?
- “Caution: Federal Law Prohibits the Transfer of this drug to any person other than the patient for whom it was prescribed
Basically; patient CANNOT give to anyone
